Sick (fit) notes

Fit Notes

If you require an isolation note due to coronavirus please use this form.

Issuing a Fit Note

Your doctor will only issue a Fit Note if your health affects your fitness for work.

You do not need a Fit Note from your doctor for an absence of seven calendar days or less. In this instance, your employer can provide you with a self certification 2 form .

Please see the following website for more information.

Fit Note Rules

You should ask for a Fit Note from the doctor who is treating you. This will usually be your GP, but if you are receiving treatment post-operatively at a hospital, you should request this from your hospital doctor.

The Fit Note is classed as advice from your doctor and your employer will decide whether to accept this or not. Unfortunately, your doctor does not have a role in potential disputes between you and your employer.

A Fit Note cannot be issued for a date in the future, however Fit Notes can be backdated, if it is a continuation of a previous Fit Note.

Returning to work

You should go back to work as soon as you feel able to and with your employer’s agreement, which may be before your Fit Note runs out.

You do not need to go back to see your doctor before going back to work. Your doctor cannot give you a Fit Note stating that you are ‘fit for work’.
