General advice on lifestyle, diet, well person clinic by appointment with nurse
Chronic Disease Clinics including heart disease, stroke, diabetes, asthma, chronic respiratory disease and as well as help stopping smoking. Appointments are made at reception.
Citizens Advice Bureau at Blidworth Wednesday Mornings
Counsellor – sees patients referred by the GP’s from both sites. This service is based at Blidworth.
Non-urgent advice: Home Visits
Housebound is defined as: A patient is housebound if they are unable to leave their home at all, or if they require significant assistance to leave the house due to illness, frailty, surgery, disability, mental ill-health, or nearing the end of life.
If you, your carers or relatives feel you are in need of a home visit as you have a medical problem then please call the practice on 01623 700805 before 10am Monday- Friday. The GP will then where appropriate visit or send the acute home visiting team.
A clinician will only visit if you are within the GP catchment area. If you have registered with us and are out of area you would not be initialled to home visits.
